1. Name of Facility.

2. Describe the implementation of the ground-water conservation procedure and approximate date the procedure was implemented.

3. Estimate/calculate the amount of ground-water conservation affected by the procedure described (may be expressed as gallons per day, million gallons per day, monthly or annual amounts, or gallons per unit of production).

4. Describe any community outreach efforts or any other pertinent items that were part of this project on attachments.
